Optional Google Analytics is loaded only after analytics consent through the site notice. The contact form is processed through Formspree. Use of these features is also described in theprivacy policy. Do not submit medical records or sensitive health information through the general contact form.

Permitted use

You may use the site for personal, non-commercial research. Site text, original tables, graphics, branding, and illustrations may not be republished, scraped for commercial republication, or presented as another organization’s work without permission, except where applicable law permits.
